Important Patient Information About Nutrition Services

Are you planning to use your insurance? Here’s what you need to know.

  1. As soon as possible, please send us a copy of your insurance card (front/back) with your date of birth. Fax it here: 206.331.4193. We need this three days before your first appt.
  2. Contact your insurance company before your first visit. It is beneficial to you to understand your insurance benefits because you are responsible for anything your insurance does not pay.
  3. Here are questions to help guide your call to your insurance company.
  • How many nutrition visits you are permitted with your plan?
  • What are your co-pays/co-insurance, if any?
  • How much is your deductible, and how much of it have you met?
  • Do you need a physician’s referral to see a Registered Dietitian Nutritionist?
  • Does your plan cover nutrition for medical, preventive, or both?
  • What is the reference number for your call? If you need it, it can save you time, money and headache.

We will contact your insurance company to verify your visit before your first visit. But we cannot interpret your benefits for you – this is something we urge you to understand.

Insurance Billing

We accept most forms of private insurance. It is our policy to submit claims to third-party payers in a timely manner as a courtesy to patients and in an effort to receive prompt payment for services rendered. We will bill your insurance company; however, you are responsible for any outstanding balances not covered by your plan. If we do not receive payment within 45 days, the full balance will become your responsibility.

In order for us to have the correct insurance information, please bring your insurance card to your visit. We accept Bitcoin, cash, personal checks, Visa, and Mastercard.


Co-payments are your responsibility as a patient. Payment is due at the time of service. If co-payment is not made at the time of service, a $10 fee will be charged which is your responsibility and is not covered by insurance.

Missed Appointments

There is a $75 charge for missed appointments. If appointments are cancelled less than 24-hours in advance, this is considered a missed appointment. These are your responsibility as a patient and are not covered by insurance.

Late Arrivals

If you are late arriving for your appointment, we will make every attempt to serve your needs, as time allows and being respectful of previously-scheduled appointments. You may be given the option to wait or invited to reschedule to a time that is more convenient for you.

Collections Policy

If it becomes necessary to transfer unpaid balances to a collection agency, any legal fees associated with those collections will be your full responsibility.

Returned Checks

If checks are returned for non-payment, you will be charged a $30 fee for related expenses.
Nutrition appointments are located in Lynnwood, WA, Snohomish County. Some appointments may be eligible for long-distance services.
